Assist clients with barriers to a completed and ideally approved PWD application
Complete PWD assessments
What do you do?
Come down to the centre at #209-304 Martin Street.
Bring three things with you:
- SIN Card
- Care Card
- PWD Application Form (booklet)
Meet with an intake worker who will take some of your info and book you an appointment with the Disability Advocate, who will fill out the assessment portion of your booklet.
1:00 to 4:00 Monday to Thursday
- No drop-in second Thursday of every month
