Rights and Responsibilities

Rights

All Access Centre Participants (individuals and/or families) have the right to:

  1. Give informed consent for service and release of information;
  2. Self-determination and to direct goals of Access Centre support;
  3. Be listened to, ask questions, and express concerns;
  4. Provide feedback and suggestions about the centre and programs offered;
  5. Withdraw consent at any time and request a copy of their file;
  6. Receive timely, caring, professional, and ethical services. A copy of the Society’s Code of Ethics that govern our practices is available upon request;
  7. Express opinions, values, and beliefs, and have them considered in service provision and anything that affects them. This includes the right to express and practice religious and spiritual beliefs while receiving our society’s services.
  8. File a complaint if they feel that their rights have not been respected or their concerns are not being addressed. (Please see complaints procedure).
  9. Have a support person or involve family in services they receive, where appropriate.
  10. Have information we collect as part of the services they receive kept in a secure, private and confidential manner. This includes the right to access their information in a timely manner.
  11. Be treated with respect and dignity.
  12. Have the basic expectations for receiving our services and the factors that might result in discharge or termination explained to them.
  13. Have cultural heritage recognized, respected, and responded to, as appropriate to the services they are receiving.
  14. Have a healthy and safe environment in which to attain services; and
  15. Have choice in service delivery such as in person at our offices, telephone service, or zoom call.

Responsibilities

All Access Centre Participants (individuals and/or families) have the responsibility to:

  1. Provide information that is true and complete to the best of your ability;
  2. Inform advocate of any new and relevant information as it pertains to your file or services your receiving;
  3. Obtain necessary information pertinent to your case and doing some of the work to promote your own empowerment;
  4. Attending meetings or scheduled appointments on time, or contacting the office if you will be late or need to reschedule and/or cancel;
  5. Adhere to the right of confidentiality of others utilizing this service;
  6. Treat staff members, volunteers, and others using the space with dignity and respect;
  7. Let staff member know when you do not understand what they are saying;
  8. Comply with requests to wear masks and/or receive service in a physically distanced room;
  9. Help the centre provide a safe environment by telling staff when you see unsafe conditions; and
  10. Responsibility to inform reception if you have a cough, fever, or flu like symptoms.
